Immunization/Vaccine Requirements
The State of Texas requires students entering Pre-K, Kindergarten, and 7th grade to be up to date on their immunizations. Having accurate records on file helps us ensure a safe and healthy environment for all students from the very first day of school.

Immunization Requirements for K-12 (English and Spanish)
Recommended vaccines:
- Seasonal Influenza (Flu) every year
- Second Varicella (chickenpox) at 4 years old
- HPV 2-3 doses
What is a Valid Immunization Record?
A valid immunization record will be required for enrolling in school. A valid immunization record must contain the following:
- Name of healthcare provider or clinic
- Your child’s full name and birth date
- Immunization names along with dates given
All records are reviewed by our Health Services staff. If any updates are needed, we will contact you directly with clear next steps, so everything is ready before the first day of school. If you have any questions, contact your school’s nurse.
Exemption from Immunization Requirements
- Chapter §97.62 of the Texas Administrative Code (TAC) describes the conditions under which individuals can seek exemptions from Texas immunization requirements. Exclusions from compliance are allowable on an individual basis for medical contraindications, reasons of conscience, including a religious belief, and active duty with the armed forces of the United States.

- Conscientious Exemption: A student may not be enrolled in school until the original, notarized affidavit is presented to the school nurse. Request the forms as early as possible to allow time for them to be mailed to you and completed before school starts. The school cannot accept copies of the form; therefore, be sure to request as many original forms as you need to provide to daycares, camps, or any outside activities. Weather Guidelines
Part of daily routine in schools is to monitor the weather in order to plan for student’s playtime, field trips and educational activities. The guidelines discussed below are based on the National Weather Service and will provide information to teachers and staff as they plan for educational and recreational activities throughout the school day.

Telehealth Brings Medical Services to Schools
Uplift Education partners with the Children’s Hospital of Dallas to offer students on-campus medical services from a doctor or nurse practitioner using video technology.
